I use this with children that are struggling with behavior on an ongoing basis. I print this form off back to back and have the children keep it in their folder. This goes to and from school each day. The child and I keep track of the positive things they did that day and then set goals for how they are going to improve their behavior the following day. This conversation helps me build a strong relationship with the child as well as communicate clearly with parents daily.
