This document will allow you to collect baseline data for the following articulation sounds:

SLP IEP Cheat Sheet (Microsoft Fillable Form)
This document can be edited! This is another document I utilize in my student data folders (GREAT for when you have a sub as well).
This is a great way to track the following:
1. Student's personal information (Bday, IEP due date, ER due date, Allergies, Medical info, Medicaid, etc)
2. Contact information (Name and Number)
3. Speech/Language Area of treatment
4. Speech Therapy scheduled days/times
5. Frequency/Minutes per month
6. IEP Goals List
This form is an excel spread sheet. It has been used to track data for speech and language goals in the school setting. There are cells to write in the goals you are working on and then to track data for each session as well as to record that sessions date.
Speech Schedule Template (editable):
This is the document I start my year with. I merge and center boxes accordingly to fill in my schedule, but first I print out this template and fill it out with pencil accordingly. Then I update the excel schedule by merging and centering blocks of time and entering student names.

Speech Homework letter (I utilize this in conjunction with my Homework Calendars) - I copy this letter on the back of each homework calendar. It gives the parents instructions for how to complete each of the different calendars and what the student is to be working on.
