Hang this on your door when administering a test. I keep a laminated sign hanging on the inside of my door so that I can quickly put it on the outside. I also have some signs printed on heavy cardstock so I can hang them outside of my classroom so they stick out into the hall making them easy to see. Have students take responsibility for their forgotten assignments. Make copies of the first sheet to keep in a binder (hopefully it won't be too full by the end of the year!). Students fill out a line for each missing assignment. They also take a half sheet to fill out to bring home with them. I use this when a student's behavior consistently does not allow them to be with their peers. I use this to document the amount of time they are away from their peers. There is space to write notes about the time away. I calculate the percentage of the day the student is away from their peers. This can be helpful when developing a Behavior Intervention Plan.
