This is a fun survey that you can use to get to know your students and how they feel about different areas of their home and school life. It is not writing intensive so all students will have access to share their thoughts. I've used humor to engage the students with an inventory that is short enough that you can review a class and hit the ground running with your group!

My students have greatly enjoyed creating with this learning task. They spend time exploring lyrics in music and the message they convey. They then explore their own creative expressions of positive and healthy messages through music video parodies. My students have created some clever and witty parodies in the past. Skittles up! I used this chart in my grade 5 class one year as a program for student character development. Students sat in pods and were encouraged to proceed up the chart as a group by practising positive student behaviors. I also created posters with their pictures to recognize their achievements.
