Students use this document to check off routines and tasks as they complete them. I have included one version with the tasks filled out and a version that can be filled in with your own desired tasks. This is a helpful tool to encourage students to be organized. Monthly Reading Logs for the whole school year! A book report template is also included with a rubric. Students choose three books from each log to reach a "Tic-Tac-Toe" each month. They complete one book report each month for accountability. Book choices that are included in the logs are ideal for 3rd and 4th grade students and include fiction and nonfiction choices. A two-fold behavior note! This is a great way to keep the parent's informed about their child's behavior AND for the student to reflect on their choices. The student should spend time in class to fill out this form, sign it, and give to the teacher for review. The teacher will sign the form, and send it home for the student to show their parents and return the next school day with their parent's signature to show they had a discussion with their parent. These flashcards cover multiplication facts from 0 - 12. The answers are not included. I have my students use this template to create their own flash cards to use for practice at home.
