Conferences can be a stressful time, so I created some forms to help ease teachers' minds! This file includes:

The summary sheets that are provided are aligned to 5th grade CCSS and Georgia Performance Standards. However, the skills are editable to fit your particular standards.
This 30-page unit includes 5 lessons and activities geared towards 5th grade, but could be modified for 4th-6th grade. It includes all foldable examples, anticipatory set, procedures, and closures for each lesson, homework, labs, directions for centers, materials list, links to relevant videos, a pre-test and post-test, unit map, and printables necessary for the unit. You may modify the unit to better fit your personal needs or send me a message and I'll see if I can whip something up for you.
Headers and strategies to go along with the 6 (or 6+1) Traits of Writing model. The file also includes blank lined pieces for you to write additional strategies on. This does not include a full year's worth of strategies, just a sampling to get you started. Use your writing minilessons and students' needs to decide on your strategies.
This PowerPoint is a great introduction to adding mixed numbers with unlike denominators. I used it as a minilesson my 5th graders before math workshop. During the presentation, I like to walk around the room while the students solve problems on white boards. My AP was observing me during this lesson and she loved it!

This is a simple and EDITABLE weekly lesson planner to help keep you organized and on track! The zipped file includes a PDF if you want to just print and go and a DOC file for you to customize.

Many districts are required to post their learning objectives or essential questions during the week. Print these out, laminate them, or hang them in cute frames, and use a wet or dry erase marker to write your daily objectives! I like to write them in the morning and erase them in the afternoon. Easy!
I've included two designs to choose from with several color choices, including blackline masters!
This file includes 2.5 inch polka dot square frames with letters (A-Z), numbers (1-10), and some commonly used punctuation. There are 11 colors (red, orange, yellow, green, turquoise, blue, royal blue, pink, purple, black, and white). These letters and numbers would be perfect for labeling, bulletin boards, Boggle or Math It Up! boards, and anything else you can dream up!

This EDITABLE PowerPoint reviews comparing and contrasting, author's purpose, inferring with supporting evidence, main idea and supporting details, summarizing, question-answer relationship, cause and effect, and briefly touched on sequencing. I used this as a quick review over two days (about 15 minutes each day) during our literacy time to review for the CRCT for fifth graders, but would probably work for grades 3-6.

ELA Test Prep, English Language Arts, Reading Strategies
This is an easy to put together center! Students can play in pairs or in small groups. Created in black and white for ink-saving printing! Students lay all cards face down and take turns picking two cards. If they match, they keep them. Student with the most matches wins. Cards are self-checking with identical symbols on all matches.

A list of 17 writing prompts to get kids' brains working! We taped these inside of our writing notebooks so students would always have it for a reference. These work well for jumpstarting students' ideas and journal entries.
Two forms to help you get through the beginning of the year: student info sheet and a supply list. I've included an editable version and a PDF if you want to just print and go.
If you're anything like me, you've probably got a running to-do list a mile long! I print these out and keep them in my planning binder. The list is separated into four columns - When, What, Who, Done.
