I use this rubric to teach my students productive collaboration skills. I have used it with 8th-12th graders. This is how I do my classwork grades. This is a simple parent intake form that could be used at an open house or curriculum night to get some information on your new students and their parents.

This activity asks students to create multiple choice answer options for open response questions. I used it in my class with gridded response questions from standardized tests. The aim of this activity is to get students thinking about how multiple choice answers are created from common mistakes.
