This is a note meant for students to complete for their parents. This is the students reflection over their day. Staff can complete or help complete as needed. The pictures are great for pre-readers. There are two options,1 page or 2 pages with larger pictures pdf documents. You can print these out and send home daily for documentation purposes. Or you can print one per student and laminate it. I really like this behavior chart because I don't have to worry about keeping the correct month printed. That may seem like no big deal. But this is just one less thing I have to add to my to do list every month. I keep this simple chart on hand, every month the kids get a fresh chart and they fill out the month and the days.

I actually have a Google Drive folder and each student has their own communication log saved in that folder. You can save this power point file to your computer and then drag and drop into google drive and open it as a Google Slide if you would like to do that.
