I use this sheet as a way for students to make up missed problems on tests. In my self-contained regular education 8th grade class, I find that if students have the opportunity to make up missed problems, they really learn a lot. I require my students to write the number of the problem they missed, the new correct answer, and explain their mistake.
I created this accountability binder to help my students keep track of their progress. I conference with my students to make sure their data matches up with my records. These binders help students set goals, track progress, and understand what is expected of them. These binders can be used during student led conferences. This is a great back to school activity. Students go around the room and look for students who fit each category. It is great because students really have to talk to each other to figure out the answers.
I tell my students they must use everyone's name once before they repeat.
I use this when I loop with a class and have most of the same students. It allows students to learn something new about classmates they know well and have known for a long time.
