This product has two different behavior plan templates; one is based off of the ABA approach to behavioral interventions and one is a simpler version that I've found to be more straight forward. I have included both pdf and writable Word files. I find that using a print out of the pdf during team meetings is nice, because everyone can have a blank copy, and we are not all looking at our laptop screens instead of the rest of the team, especially if the parent/guardian and their child are included
This item contains three different elementary level think sheets, each with a slightly different focus to fit the needs of your classroom. Included are: - an ABA style think sheet - a Restorative Justice style think sheet - a Simple format think sheet The ABA-style Think Sheet helps the student identify the target behavior, why they engaged in the behavior, and strategies that could help next time. The Restorative Justice Think Sheet has the student identify who they harmed, how the harmed per

This is four versions of the decision tree reflection sheets I use in my classroom. Of all of the reflection sheets I use I find these to be the most effective. Instead of having the student reflect on what they did wrong, these sheets have the student create a set of options they can choose from and analyze the possible consequences for each set of options. This can be done while the student is currently considering making a negative choice (for instance, I have had students complete a sheet wh
This produce has four versions of a middle- to high-school level behavior contract. Included are versions where the student must brainstorm their own behavior to correct and versions with pre-chosen behaviors that the student can check-off. In all versions of the contract the student must identify strategies they can use to ensure they fulfill their promise. There are also two versions where the teacher can include strategies they use in the classroom that will help the student meet their goal.
Included are the following: - a "simple" goal sheet: this sheet has a straight forward format for students who already know how to create an attainable, relevant, and clear goal for themselves - a "strategies" goal sheet: after creating a goal, this sheet requires students to consider and record strategies that will help them achieve their goal - a SMART goals sheet: this sheet gives students more prompting to follow the SMART goal process to create their behavioral goal
