This is the template I use for substitutes any time I have to be out. Everything in bold red is personal information related to your class.
You could edit this to make it very generic and put it in sheet protectors for a sub binder or edit and print hard copies each time you have a sub and place in a folder with copies for the day.

I keep multiple copies of this sheet on a clipboard at my kidney table, and rotate when it fills up. Resource includes notes on the information to include, a sample with a filled-out section, and a blank template.
These resources are useful to young writers who need to see a strong visual as they create final products. The first is a 5 paragraph essay structure so that students know where to put the title, transition words, details and conclusion. This Excel sheet has been useful as a daily check for student folders and homework. Put dates in the top row, student names in the left column, and teacher names on the tabs. There are 3 teacher tabs and 30 student lines assigned to each teacher, but the document is fully editable.
-If a student has brought their homework and folder, put an "x" in the corresponding square. It will turn green.
-If they are absent, put an "a" in the corresponding square and it will turn white.
