After experimenting with a large variety of guided reading lesson plan templates and note-taking sheets, I have boiled it down to this one. This document includes: -Student Names & Independent Levels -Instructional Level of Selected Text -Title of Selected Text -Note-Taking Section I keep multiple copies of this sheet on a clipboard at my kidney table, and rotate when it fills up. Resource includes notes on the information to include, a sample with a filled-out section, and a blank template.
