I created and utilized this goal sheet when I was a grades 4-8 resource teacher. This was a good way for me to create a dialogue about goal setting, help my students become more self aware, and motivated. The form is editable for you to change on a regular basis. I keep it on my One Drive / Google Doc and it is easy for me to make a new weekly lesson plan. This allows me to copy and paste, and plan weeks at a time with ease.
