Before meeting with a general education teacher to discuss a student, I always come prepared with a specific set of questions. These questions are used so that you can quickly get all of the information that you might need for an upcoming IEP.
This was mostly used, when I was a special education teacher, used with teachers on a departmental schedule. I have also used this in the primary education as well.

As a Special Education Teacher or ELL Teacher collaborating with general education teachers about students prior to the end of grading periods or IEP/ILP conferences can be difficult. Especially if you are following a departmental schedule. This editable form is great to keep yourself organized. You can print the form and write on it, type on it, or send it to the general education teacher to fill out.
I utilized this as a special education teacher, and as a general education teacher. This was a way for me to sit down with each student, and just get to know them. As I was asking them the questions, I would write down their responses and evaluate their listening comprehension as well as their communication skills. You could also use this at the beginning of the school year as a way for parents and students to sit down and talk.
