Behavior Frequency Data Sheet (Editable)
This data sheets allows you to track the frequency and time that behaviors are occurring. This is helpful for tracking ongoing problem behavior and/or when conducting FBAs. This is a Google Slides file, so you can edit the student's initials, behaviors, and times/intervals. To use, edit the relevant information and use tally marks to indicate when a behavior occurred during each interval.

Individual Child Planning Form
This form is for use in a preschool or kindergarten classroom to help teachers and staff plan for child-centered and project based learning. I recommend printing a separate sheet for each student and keeping them in a binder or displayed on a bulletin board with easy access. I find that the best way to keep parents on your side as an educator is to keep them informed of what is happening in your class. Each child that tattle tales will have to fill out this report and file it in the tattle tale report box.
