Basic Skills Checklist for Kinder
I use this checklist to check off what students know or don't know yet. Using pencil allows me to erase and make changes as the year progresses.
I can also change skills, or words, each year if necessary, and I can use this as a goal-setting tool with each student.
I provide a copy to parents at the end of each 9 weeks so that they know where their child is and where he/she is headed since most of this detailed information is not part of our report card. Weaknesses can be seen at a quick glan

Pink Slip - for missing homework
This is a strategy I got from Harry Wong (great presenter and author!). It's called a Pink Slip, and students fill it out whenever they do not have their homework. I don't listen to excuses, I just have them fill out the form, and I'll look at it later to see what their excuse is. I keep these on file, and they have come in handy for parent conferences (especially if a student is saying they didn't have time to do their homework due to sports or something or the sort).

Use this self-evaluation to help your students conduct student-lead parent teacher conferences. Your students will be encouraged to reflect on their behavior and study habits in class. Missing Homework Excuse Form for Students
Last year I created a "form" for students to fill-out when they did not have their weekly Homework ready to turn in on Friday.
This helped me when conferencing with parents. When the child had zeros on a progress report, I could easily take out the HW excuse forms filled in by the student. It did help the students feel more accountable.
