What do you have your kids work on when they are in timeout? Have them fill out these forms to give you more information about the behavior situation. Have the younger kids draw pictures if they are not yet able to write. Once the kids draw the picture, you can dictate for them and write their thoughts next to the pictures. This form also allows you to check for the child's emotional state and has them think about whether or not they need to apologize for their behavior.

This simple, half-sheet, daily report works well for students that have difficulty with other class-wide behavior systems and can be tailored to the child's individual goals.
