Struggling to fill your conference time with parents? Here is a great way for students to get engaged in that time with their own reflection of how they feel they are doing in each of their classes. Have students fill out this form the week before conferences, then have it ready to give to parents when you meet with them. Have parents keep it, save it, take it home, and refer back to it with their student when they want.

This is a great graphic organizer (front and back) with space for a "DO NOW" and Exit Slip question Monday-Friday. The back of the sheet also includes a self-reflection on the week for effort and behavior. This can be used for a weekly grade and as both a formative and summative assessment of learning. I found it a very useful way to start my middle school classes, but could see this as something to implement from grades 3-12.
