Concept Checklist and Tracker for Quizzes - Growth Mindset and Mastery

Inspired by Dan Meyer's concept checklist! Every quiz is numbered and has the concept written at the top. For example: #16 Solving absolute value equations. Students can score a 1-4. Students can retake the quizzes as many times as they need to! The student fills in the larger bubbles as they improve. For students to want mastery, I give them a quiz with a more complex problems and if they get that correct they get a physical gold star sticker. This is a great way to promote the growth mindset a

This sheet is a great way for students to reflect in their performance and help them set targeted goals for themselves! This should be given to them after a mid-unit quiz so they can set goals for their performance on a unit assessment. You can see on the second page, or like I use it: the back of the sheet, I provide the standard(s) being assessed and instead of setting a goal like "I will improve from 60% to 80% in standard 7.EE.B.4a by....." students should use the description and verbiage of

Quiz Re-Test Form

If you offer re-tests for students who do poorly, this is a useful form for students to fill out ahead of time. On this form: - It covers basic info about the quiz (when it was, the topic, student original score, etc) - It asks students to explain why they did not do well the first time - It holds them accountable for doing more work to improve their understanding of the material and document what they did (study using quizlet, complete missing assignments, meet with teacher or tutor, etc.) -

Sample Corrective Behavior Quiz

At times our students are not focused throughout our lessons. One day I had to get creative because I had a few students off task after being redirected multiple times. The next day they had to complete a quiz to ensure that they learned the content in the mist of the talking and/or horse playing. I have only had to use this quiz a few times over the past three years. Once students realize that they are being held responsible for the content whether they focus or not, they quickly correct their

It isn't one of those things that you try and then it doesn't work or the kids don't like it. Or you all just get tired of it. It's a little Energizer Bunny to keep the vocabulary work happening. You can pull names to put a student vocabulary card on display on a document camera or copy and enlarge them, then put the best ones on a word wall. For a quiz, put po

Assessment Analyzation

Use this graphic organizer to help you analyze student performance on any quiz, test, project, or assignment. I begin by grading every assignment, then calculating the measures of central tendency (mean, median, mode, etc.). Then, I write each student's name and percentage in the appropriate column. This helps me see if the students' scores form a bell curve around the B-C range, or if they're skewed left or right. Below the table, I write down the name of each student who earned a D or F, a

After a quiz, these sheets should be made available to students to take for each problem they got wrong on a quiz, so that they can take time to think about what they did wrong and then correct the problem. There is a written justification involved so that the teacher can see how well they truly understand what they did wrong and award points according to how well the
