Problem Solving Graphic Organizer

Students learn to think their way through problem solving with this graphic organizer. I make a ton of copies of this at the beginning of the year. While I know our copies are precious, I have found it to be worth it. Once students are familiar with the process, they don't have to use it as much or they use a short hand version on scrap paper. I also always keep a stack in my problem solving center. If your worried about copies, laminating them and using the thin vis-a-vis markers work wonderf

This sheet includes self-reflection questions that ask students to think about a negative behavior they have engaged in and respond to their reasoning behind the incident and what they can do to prevent future outbursts. Sheets can be kept by teachers as a means of tracking student behavior and make excellent talking points if a parent-teacher conference/message/phone call is made home.

Student-completed Behavior Monitoring Checklist

I use this checklist for data collection and student self monitoring. Prior to class, I discuss the process with the student. If I am seeing a disruptive behavior during class, I will grab a pre-printed checklist, write the behavior on the top of the sheet (for instance, blurting or getting out of seat) and place it in front of the student. If the behavior continues, I will simply point or nod to the student every time the behavior occurs. The student should then mark down every instance. Af

This Word Form helps plan an essay with a claim, a logical and physical design, and an outline of sub-arguments; students can fill in the fields, or instructor can work through the steps to model the process that a thoughtful essay-writer goes through during the invention process. The idea is to help develop a habit-of-mind to design essays with more meaning and depth. This kind of brainstorming and reflective thinking should happen early in the process so that the actual writing of an essay is

Self Assessment

Children can learn how to evaluate their work and reflect on it in a critical way. If you have been collecting students' work, have them look through their portfolio and select the items they are most proud of. Teaching students to go through this process helps them hone their skills when they write and edit their work. I came across these assessment sheets in my files. I do not know where they came from so I am posting them for free. If you have seen these before and know their origins, please

Special Education Behavior Assistance Forms

Use these forms when a student is in need of intervention from office or professional personnel. Print the forms out on two different colors of paper, and hang them at the door of each classroom where they may be needed (or taken out to the playground during recess). In an emergency a student or para-professional can rip a form and take it to the office. The objective of these forms are to communicate to the office as quickly as possible what is happening in your classroom (without you needing t
