Missing Homework Record
Use this to help keep track of missing homework.
I keep it in a binder, in numerical order. (My students all have numbers, but this could easily be done alpha by last name.) When I'm collecting homework, any student who does not have their homework is required to fill this out. On Friday, I quickly scan through the binder and e-mail home the parents of students with missing homework. This keeps everyone "in the loop" and on the same page.

i-Ready Diagnostic Data Tracking Sheet for Data Binders - i-Ready Goals
This sheet can be used for any grade level and for either ELA or Math i-Ready data. There is a place for the students to record their Diagnostic scores in the beginning, middle, and and of the year. It has a place for their individual goals also. The line next to the fall beginning of the year score also asks for the grade level equivalency for that score. Pink Slip - for missing homework
This is a strategy I got from Harry Wong (great presenter and author!). It's called a Pink Slip, and students fill it out whenever they do not have their homework. I don't listen to excuses, I just have them fill out the form, and I'll look at it later to see what their excuse is. I keep these on file, and they have come in handy for parent conferences (especially if a student is saying they didn't have time to do their homework due to sports or something or the sort).

Student Schedule for Beginning of school year
At the beginning of every year, I like to give each student and parent a copy of the weekly schedule. Some schedules are tougher for students than others, being that some years we have had 6 versus 5 days.
