RTI - Graph for Student Recording Daily Independent Computer Tutorial Programs
I use this to help students track their learning by recording daily scores and then graphing them. It could be used with any daily computer or workbook program. I put it in their center folder and it keeps a great running record and visual record of their progress. We have a big emphasis on students becoming responsible for their learning and this tool helps them see how they really are doing. We have discussions from time to time and the student explains what they see on the graph to me.

Computer Login Cards with Multiple Program
The Computer Login Cards were created with the programs available to me at my school. These cards allow my students to have all of their information at their fingertips. A card was sent home, given to the student, and kept in the classroom. The programs can be edited to fit your school's available programs. This color coded document will help keep you on track with the second grade Common Core enVision curriculum. Topics are color coded by the semester they are taught so you can remember what topics you covered, when you covered them, and quickly see what topics are coming up.

iReady Test: Data Tracker Sheet to Set Goals & Gauge Growth {Reward w/ Grade}
While students sit down at their computers to take the iReady test, give them a reminder of how they did on their last diagnostic. This helps them set goals and get immediate feedback on how well they did relative to their previous performance. It also sets the tone that this is a serious test and they should try their very best!

Missing Assignment Form1
When assignments are due, inevitably there are always a handful of kiddos without the work. Use this form to hold them accountable for completing the (now) late work and it will also serves as a great way for you to have documentation. As I tell my students, "you must turn something into me....either the assigned homework or the missing assignment form."
This can be easily modified for any subject or grade level.

Problem Solving Document for teachers and a blank for students
This walks students through what the operations do in math, not using code words. My students use this when solving word problems. We go through this as we introduce the operations in word problems. We fill out an anchor chart and we spend a lot of time discussing what the operations mean. I have a blank copy for the students and have included a copy filled out for you to adjust as needed.
