Missed test or quiz form

I use this form when a student is absent and needs to make up a test or quiz. I complete the top and put it in the "absent work" box for the student. The student picks it up and completes the rest. They tear off the bottom for a reminder and submit the top to me. I staple it to the test and put it aside in a designated folder so I dont have to search for the test or quiz at the last minute.
