I created this "Glow" and "Grow" form to pass out to classroom teachers for students that I do interventions with. This helps to have a "conversation starter" with parents and list observations from small group.
"Glow" would be a list of things that the student shines or excels at and "Grow" would be a list of things that the student needs to work on.
