This is a tool that students can use to self assess their work habits and learning skills. It is written is student friendly language. The self assessment is of six different learning skills: Responsibility, Organization, Independent Work, Collaboration, Initiative, and Self-Regulation. The rubric may be used small portions at a time where students select one learning skill to reflect upon at a time. This rubric may also be used in Parent-Teacher or Student Led conferences.

What are your students' attitudes towards reading? Do they read at home? Do they go to the public library? Find out all you ever needed to know about their reading interests and habits in 20 simple short answer questions. Great to use as a diagnostic, or at any point throughout the year. I would store it in their portfolios and have them review their responses at the end of the year to see how much they have changed over the year.
