Since it is a pain to rewrite conference letters each time they roll around, I use the same letter, but edit the dates and times!
I use the sign up sheet not only to figure times that each parent can meet, but also as my own reference of the conference schedule.
I've found this method particularly successful, but if its not exactly what you want, just remember that it is completely editable to your own conference
These rewards are totally editable to fit your classroom. My students save up "Star lake Way" tickets to purchase items in our classroom. When they saved up the amount shown on the reward, they can purchase the reward card and use it when they like!

I use these math fact tracking sheets for students to log their math facts every day when doing timed facts tests. The assignment portion of the log allows you to put which facts they are working on, whether its multiplication, division etc. My favorite part is the space where students can write the facts they missed on the test. This helps them reflect on which facts they need to work on. Over time, students really gain ownership of their facts and start to see trends in their growth!
This editable behavior self reflection gives a space for students to score themselves on each criteria as well as the teacher to give them a score. This is such a great resource for conferences, since it really shows how the students view their behavior compared to how the teacher does. I usually use the following scoring system: 1- Rarely/Never/Needs a lot of reminders 2- Sometimes/Needs some reminders 3 - Usually/Most of the time/ Rarely needs reminders 4- All the time/ Never needs reminders
