This is a daily behavioral point sheet I use in the middle school setting to help students manage their behavioral goals.
Students carry it in a folder and the teacher in each bell completes it and initials- I total their points at the end of each day. Students earn a maximum of 3 points per objective per bell and have a daily percentage goal to meet to earn a reward. These "task cards" are designed to be taped to each student’s desk to enable them to self-monitor their behavior during each class period. Laminate or fully cover with packing tape!

This is a modified Daily Behavioral Point sheet I created using Boardmaker pictures for a student with Autism. I use smiley face stickers in the squares- the student receives a sticker for each objective (from IEP) for each bell of the day. This motivates her to maintain her behavior throughout the day!
I print this and put it in her traveling behavior folder she brings to her classes and tape a sheet of stickers beside it (stamps also work!) If she earns at least 25 of the 30 possible stick
