This data sheet is one I created and used to collect data for a student on my caseload who eloped (left class suddenly and without permission) frequently, didn't complete work, was aggressive, and would be non-compliant toward most directions. Giving accomodation sheets to resource and classroom teachers should always be documented by special education teachers and case managers at the beginning of the school year. The same is true for collecting the documentation at the end of the year. These sheets will help keep track of doing just that. It does come up at times in IEP meetings whether or not a teacher has received documentation. The teachers name and signature can show when they received paperwork.

This sheet is for special education teachers, inclusion teachers, specialists (OT, speech, etc), and school administration. This sheet is a lifesaver for teachers to be able to look quickly and plan for the day around students' accommodations and times out of the classroom. Accommodation sheet is a word document and is editable.

Sometimes in the middle of writing IEPs it can get overwhelming and you may forget something. I created this checklist to help myself make sure I remembered everything for the meetings. This product is editable to the requirements of your state and district. I created mine for Norfolk Public Schools in Virginia's requirements.

You will be able to persoanlize your small group lists.

When a student is having a meltdown, there are times when they can't reel themselves back in independently. Having students use their senses to take their mind off of being upset and helping them to identify things around them will help students come back down from their episode. Also a great tool to use while teaching the five senses. Students could find pictures in an old magazine or draw pictures to go in each column.
