Formative Assessment Chart for All Grades and All Subjects
I teach math, and I use this form to track student learning with each lesson. (I have used it in other content areas as well.) I fill in the chapter and lesson number at the top, as well as the questions from the chapter test that correlate to the lesson. I also include the Common Core “I can” statement.

Behavior Sheet for First Grade
I use this sheet when a student (s) cannot follow the classroom rules. This form keeps the parents up to date on behaviors being seen in the classroom and how well the student is following classroom rules.
I put a sticker or star in each area so that the student knows how well he is doing. I also contact the parents to enlist their help. If the child gets all the stickers for the day, he/she goes home to a special treat from the parents.

Progress Monitoring for IEP Goals/Assessments & Data Tracking
Not many Progress Monitoring forms exist for RTI and IEP goals for PreK and Kinder students. You'll get two forms to assess letters and sounds, numbers and shapes. These progress monitoring forms allow me to progress monitor in minutes. I keep these pages in a binder with a tab for each student.

Olaf counting 20 Frames for wall decoration
Olaf counting 10 Frames for wall decoration. I drew snowflakes in the middle of them and use them for number identification when counting, spelling and identifying numbers. 1-10. How I filled this out is that I wrote the number, in the frames you show the representation of the number, and on the bottom spell out the number.
