Many of us are using the NWEA MAPS assessment to monitor our students' progress in both reading and math. This is a worksheet that I created that enables students to take ownership over their MAPS scores, and encourage them to make growth. Using this worksheet, students set goals for themselves to achieve throughout the year in both reading and math. Additionally, students can monitor their growth throughout the year to see if they have achieved their goal.

The tracker includes Texas Essential Knowledge and Skills (TEKS) for the 8th grade math curriculum. It can be a resource for teachers, students and shared with parents when filled out. The file is editable, and the fourth column can be changed but the author used it to track the "reteach" mastery. The tracker is one page long when printed with narrow margins. I made these username and password cards so my students could independently use the technology lab. I plan to fill in student information, cut them out, and laminate them. They will then be made available to students when they are ready to use the classroom, media center, or technology lab computers. They could also be sent home so students could assess internet resources after school.
