Student-completed Behavior Monitoring Checklist
I use this checklist for data collection and student self monitoring. Prior to class, I discuss the process with the student. If I am seeing a disruptive behavior during class, I will grab a pre-printed checklist, write the behavior on the top of the sheet (for instance, blurting or getting out of seat) and place it in front of the student. If the behavior continues, I will simply point or nod to the student every time the behavior occurs. The student should then mark down every instance. Af

Exit Slip: What have YOU learned today?
These exit slips are formatted within a PDF, downloadable file and have four per page.
They prompt students to process through some metacognition-ness and ask them to explain ONE thing that they have learned today..doesn't even have to be in your specific class.
Additionally, they require students to FOLLOW DIRECTIONS and to recall a moment of learning individually.
I place these exit slips in a pile on their table of desks prior to the school day, which eliminates taking time to pass them

Children can learn how to evaluate their work and reflect on it in a critical way. If you have been collecting students' work, have them look through their portfolio and select the items they are most proud of. Teaching students to go through this process helps them hone their skills when they write and edit their work. I came across these assessment sheets in my files.
