This is the google form I use for my concert/ rehearsal attendance. It is an editable google form. I like the convenience of it being electronic and timestamped. I have students scan a QR code (posted by the entrance) and fill out the form on their phones. I have 2 laptops available for students who don't have a device. I recommend sharing this form with your students via a QR code for easy sharing.

The behavior card is a quick way to communicate with the primary teacher in elementary school and parents/guardians about student behavior during music class. Mark the box next to the expectation the student didn’t meet. I use a color coded system with white being the first sent home, yellow the second sent home, and pink the 3rd (or more) time sent home.
