Checkpoints are my version of a formative assessment. This one is 10 questions all based on the common core standards dealing with proportions, unit rates, similar figures, and converting measurements. There is an I can statement at the top as well as a spot for you to mark whether the student is proficient or nonproficient.

I give each student these forms to hole-punch and put in their 3 ring binder that they have. You can keep them in a data binder or folder as well. After each assessment over the standards (some of my assessments include more than 1 standard), I have the students color in the block that matches that standard. This allows me to see which standards I need to intervene on for each student. (I also record the standards in my own data binder for each kiddo, in case they lose their sheet.). The stu
