Based on a problem solving chart I received at a workshop, I created a journal/notebook style guide to help students fully explain the process of solving an exemplar.

Background
I have a student binder with the name of each of my students on a tab. This is where all the data ends up: conferencing pages, running record charts, etc. In the front of the binder I keep the old weekly planning charts. In my grade book binder I place the classroom observations to make grades easier at the end of the trimester.
