Parent Update/Newsletter: "What we worked on"
This is an editable template for teachers to inform parents what they worked on daily throughout the week. You can type on it or write on it. This is great especially for special education teachers with students who complete various tasks daily. It keeps parents informed! Daily Target Behaviors Tracking Sheet
Although a student may have several behaviors that need improvement, trying to work on all of them at once can be frustrating and overwhelming. Students can manage focusing on one or two goals and as they see success, an improved behavior can be switched out for a new behavior target to work on.
This daily target sheet is easy for a teacher to manage and takes very little time. It is a nice visual reminder of what goals they are working on throughout the day as well.

I use this behavior contract with my students who need just a little bit more motivation. It is very effective if used consistently and with parent involvement. I usually have the students work towards a personal goal/reward to motivate them to follow the tasks listed on their contract (i.e.lunch with teacher, extra iPad time, etc.)

Communication and Documentation
Keep parents in the loop by having staff or students (depending on the age) fill out this daily log of what happened. This is a great way to get parents involved! It also works really well for documenting individual student progress. Also, by documenting the child's daily activities you can begin to determine patterns, struggle, and strength areas for each student.

Self-Monitoring Behavior Charts
Two different levels of self-monitoring behavior charts.
How to use:
I sit down with each student and we chose 4 behaviors to monitor throughout the day. They also chose the reinforcer they will be working towards if they earn the necessary points.
After each segment, a paraprofessional or myself review if they met this expectation. At the end of day, if they earned enough points then they get their reward!

Behavior Record
This is an editable behavior chart that allows you to customize your students' behaviors rather easily. It is a 7x8 table that the days of the week are listed at the top of each column while the lead cell in the the row is an empty box that you can type in the target behavior or behaviors that you are working on that day/week. The eighth row is left blank for comments and/or signatures.

This table is to help record a child's behavior throughout a period of time. It allows the educator to write down a specific behavior, time and date plus possible reasoning for said behavior. In the bottom I have included examples of behavior that might be of interest to be recorded. This tool is also excellent example for a parent in order to help establish a record for behavior specialist and other professionals working with the child.

This is a behavior sheet I created for my students to take home every night. It correlates with my clip chart tally mark behavior sheet. Green- 0 marks, yellow -1 marks, bottom of yellow -2 marks, orange -3 marks, bottom of orange -4 marks, and red -5 marks.
