This is an editable behavior chart that allows you to customize your students' behaviors rather easily. It is a 7x8 table that the days of the week are listed at the top of each column while the lead cell in the the row is an empty box that you can type in the target behavior or behaviors that you are working on that day/week. The eighth row is left blank for comments and/or signatures.
Your students will be able to recommend books to each other. Make multiple copies and put in a binder. Include this in your reading center and students can read to find recommendations.

This is a full day visual behavior chart organized by activity. It is aligned with PBIS strategies, and students can use this for self-monitoring. Students or teachers color the rectangle next to an activity red for inappropriate choices or green for appropriate choices. Please see page two of the document for the "Tips for Success."
This checklist is used for each test. The checklist is used to assess if there was a consistence amongst students to determine if further time needs to be spent on lesson. Student is given about a week to correct errors on test. It does not change the grade, but helps to determine if students continuously makes the same mistake. This can be used for any subject.
This is a sheet I use for difficult or distracted students to monitor their own behaviors. They check in after each subject and I give them a smiley face or a sad face. I then total up the smiles out of the number possible and the reward is given if needed. This was great for tracking for behavior RTI. The specific learner behaviors are easily customized for your needs.
