Reward and Consequence Behavior Chart

We have all been there....we have a child that is unruly and just refuses to listen and follow any rules. I have made this chart just for them. The chart is very simple to use, if the child does well in the 15 minute time span then they receive a star. After so many stars in a row (I usually do 3 stars, but it is up to you) then the child receives an immediate reward for their behavior. Now if the child is having trouble during the time span, mark an X or leave it blank so they know they need to

This editable weekly behavior chart allows students and teachers to set two goals to focus on for the week and decide upon a reward for reaching their goal 7 or more times. I have always called it Check In/Check Out, as the student checks in with the teacher periodically throughout the day to discuss the choices they have been making. At the end of the day, students receive a sticker or smiley face in the box next to any goal they achieved that day. There is also room for teachers to write a not

This weekly + monthly behavior point chart is used to manage problem behavior. It is editable so you can change the goals as needed. At the beginning of the week, the student thinks of their point goal for the week and it is written/typed at the top. If the student makes his/her goal, reward! If not, they have points (data) to reflect on and strive to beat. The points add to the total as you enter them so it saves you time! This can be used as a Tier I and Tier II behavior intervention. I developed this seating chart spreadsheet to use to group struggling students with students who understood the content on the previous assignment. I have my students in 6 different groups. You put in the scores from an exam or preview activity, and the program will match up a student with a HIGH score with a student with a LOW score and several students who fall in between. I do this after EVERY exam. My students love seeing what group they are in when they return after an exam. I will cus

I left it as a Word document so that you can modify it to fit your needs. Thanks for the download and enjoy!!!
