This product has two different versions of a student/group accountability sheet for group leaders to keep track of while their group is working. One version has an option for you to include the activities the students are completing. The other version is just a student participation sheet. All groups start off with 100%, but every time they have to get a warning from the teacher they deduct 10%. You can use this document to list your students and the accommodations that they receive. I use it as a "quick reference" instead of looking through students' IEPs and 504 plans. It's in a spreadsheet so you can edit it with your students and print it. You can check off the accommodations that your students have or type in. I included the most common accommodations that I've found in student IEPs/504 plans, but you can easily edit to input any other accommodations you find necessary.
