If you like this product, please rate it so I know to post things like it! This is my take on the ILP (Individual Learning Plan). It is the most detailed, most inclusive, and most comprehensive ILP I've ever seen. I use this ILP to document assessment data in one place and make decisions about how to meet my students' needs. I also show this document to parents during conferences to show them students' strengths and weaknesses. I have broken down subjects (Reading, Writing, Math and Life Ski

Having Students Take Initiative for their Grades

Here and there I have my students fill one of these out after looking at their grades, so that they can become more responsible. This sheet allows students to own up to why they have a bad grade, while also allowing them to ask themse
